Medication non-adherence represents a critical healthcare challenge, particularly among elderly and chronically ill patients, contributing to treatment failures, increased healthcare costs, and preventable mortality. This paper presents an innovative IoT-based Smart Pill Dispenser system that integrates ESP32 microcontroller technology, AS608 fingerprint biometric authentication, automated stepper motor dispensing mechanisms, and comprehensive real-time remote monitoring capabilities. The system employs a secure fingerprint authentication module for personalized access control, mobility, precision timing mechanisms using DS3231 RTC, and comprehensive IoT connectivity through WiFi and mobile applications for caregiver notifications. Key features include automated medication scheduling, voice-guided reminders, tamper-proof dispensing controls, and fail-safe mechanisms to prevent overdosing. The proposed architecture addresses critical gaps in existing medication management solutions by providing secure, scalable, and cost-effective automation suitable for hospitals, elderly care facilities, and domestic environments. Experimental validation demonstrates a 92 % improvement in medication adherence rates, 85 % reduction in human dispensing errors, and 99.8 % system reliability with comprehensive safety mechanisms. The modular design ensures easy integration with existing healthcare infrastructure while maintaining data security standards.
